This Federally Qualified Healthcare Center serves anyone in the community in need of affordable healthcare. Services available include Primary Medical Care, Dental Care, Medical Case Management, Pharmacy, Substance Abuse and Behavioral Health Counseling, Peer Services and Housing Case Management. Telehealth services available.

Hours:

8:30am-5:15pm, Mon-Fri;
8:30am-7:00pm, Wed

Intake Process:

Present with insurance or if uninsured, provide proof of income less than 200% of Federal Poverty Level for sliding fee scale.

Complete patient registration form.

Program Fees:

Dependent upon income if under 200% of FPL
