Description:

Mission: To promote and protect the health and safety of all people in Florida, through the delivery of quality public health services and the promotion of health care standards. Provides childhood, adolescent, and adult immunizations such as DTAP, TD, polio, hepatitis A&B, Tdap, HPV, pneumonia, chicken pox and measles. Flu vaccines are available seasonally. Program Fees:

Flu shots $20; pneumonia shots $25; free CDC recommended vaccines; birth certificates $12, $6 for each additional copy at the same time; death certificate $8

Eligibility:

Residents of Seminole County; visitors (for TB services. ID and immigration form I-94 or asylum card required if visiting); uninsured & underinsured women ages 50-64 from Seminole, Lake & Orange counties (free & low-cost mammograms and pap smears. ID, proof of income and proof of residency required)
